You can apply for Disability Insurance online, or if you are unable to complete the application online then you can apply by calling their toll free number 1-800-772-1213 between 8:00 a.m. to 7:00 p.m. Monday through Friday, their representative can make an appointment for you to apply.

  • Marriage and divorce information like spouse date of birth.
  • Names and birth date of children
  • Military service information.
  • Bank information for direct deposit
  • Your education and training information. 
  • Name and contact information for someone who can help you with your claim.
